Acknowledgement

The —— SLUSHY / HOT CHOCOLATE —- machine remains the property of Essential Equipment & Supplies at all times.
Only product approved by Essential Equipment & Supplies is to be used in the machine.
Product to be exclusively supplied by :

Terms

The machine is provided free on loan.
Cleaning is the responsibility of the venue staff.
The machine will be placed in a prominent position.
The machine is provided free on loan by Essential Equipment & Supplies with the agreement that only approved product supplied by Essential Equipment & Supplies or its distributors is to be used in the machine.
If it is found that a non-approved product is being used there will be a rental charge of $35 per week per machine dating back from the installation date.
Essential Equipment & Supplies or their distributors representatives will be provided reasonable access to inspect and maintain the machine if required.
Essential Equipment & Supplies has the right to remove the machine at any time with 24 hours notice.
The machine must be operated and maintained in accordance with the instructions as provided by Essential Equipment & Supplies.
I agree to purchase a minimum of 1 carton of each suitable approved product required with the initial order.
The machine is to remain on the venue premises at all times and is not to be removed without the prior consent of Essential Equipment & Supplies.
From the delivery date of the machine, any costs arising from accidental damage caused due to incorrect handling or total loss how so ever caused are to be borne by the operator.
